To request materials in an alternate format, please complete this form. For each book, be sure to fill out all required fields on the online form. Make sure to submit a copy of your receipt to email@example.com or deliver it to 100 Dunford Hall.
To request an Interpreter or Transcriber, please complete the form above. Requests should be made at least two weeks prior to the event. Requests submitted in less than two weeks will be accepted, and all attempts will be made to provide services. However, understand that at times, it may be impossible for ODS to accommodate a delayed request. Please review the Guidelines on Requesting Interpreting/Transcribing Services for associated fees.
Students requesting exam accommodations in the Testing Center with less than 7 days required notice must complete this form. Unless you receive confirmation from ODS, you must take your exam as scheduled in the classroom or make arrangements with your professor.
Faculty with a student with disability enrolled in their course and in need of accessible media may complete this form with as early notice as possible. Faculty is encouraged to complete the captioning request form prior to the start of the semester or two weeks prior to the date the media is to be shown.
Please complete this form if you have experienced a barrier on campus. Provide as much information as possible. The form will be reviewed by the Director of Disability Services and referred to the appropriate university officials.
