Note Taker Request

Important Note:

Before completing the Notetaker Request Form, you must do the following:

  • You must request services for the corresponding semester. Please complete the Accommodation Letter Request now. Please note that Notetaker requests will only be processed if you have received your Accommodation Letter for the corresponding semester.

  • If you drop a course, or no longer need a notetaker for a course, you must notify the office of the change as soon as possible.

  • Only list the classes for which you are requesting a notetaker

  • Once, you have submitted your requuest, please allow at least 10 business days for processing.

  • All correspondences will be sent your UTK email account.

  • Please note that you have successfully submitted your request when you receive an automatic response email from Disability Services. If you have not received an email, please contact ODS immediately.

Policies and Procedures

The following policies and procedures are applicable to you, the student, who is requesting note taking services for the current semester. Please read each statement.

  • I must request services each semester by completing a Note Taker Request Form.
  • I understand that I will be receiving my notes via email from odsnotetaking@utk.edu and NOTdirectly from the Note Taker (unless approved by ODS).
  • I understand that if I have not received notes for a class within 48 hours of the class meeting, I am to notify odsnoteaking@utk.edu for assistance.
  • I understand that if I have not received notes for a class within 1 week of the Professor making the announcement in class, it is my responsibility to contact odsnotetaking@utk.edu for assistance.
  • I will attend class regularly. I understand that notes will not be given to me if I am not in class.
  • I will notify ODS if note taking services are no longer needed.
  • If any problems arise, I will contact odsnotetaking@utk.edu or my ODS Coordinator immediately.
  • Copies of the notes received as a result of my accommodation are not to be distributed to any student.
  • I understand that note taker services are designed to supplement my learning. Having a Note Taker in class does not mean that I am not responsible for attending class, actively participating and taking notes for myself.
