Temporary Injury-Illness Registration Form

Students may experience a temporary need for accommodations due to injuries and/or short-term illnesses.  While these conditions do not necessarily rise to the level of disability, ODS will provide accommodations on a temporary basis in order to assist students who wish to remain enrolled and complete their courses.  Students requiring temporary accommodations may complete the appropriate online Temporary Injury-Illness Registration Form and contact ODS to schedule a meeting with an accommodations coordinator.
